Was given the care plan, but it wasn't really reviewed. Dr. said he would submit referral for test, first step in process and that he wanted me to see him in three months. Schedule is booked out so far, not able to schedule in clinic at the end of appointment. Was told I would be called and they would do it at that time. It was a week and I hadn't heard from anyone. I sent a message through the portal. I was contacted the next day. Referral for test had not been sent in. The doctor that signed the care plan was not the doctor I saw. We never discussed my diet, why was that on assessment and, my blood pressure was not elevated and is under control with my primary care provider. I was not there for that and my diet does not have anything to do with my symptoms. I do not have Dyspepsia. I do not take or need medication for acid reflux.
